| 1. | Your family name (sometimes called a 'surname'). |
| 2. | Your given names ('first' or 'Christian' names). |
| 3. | Your date of birth. |
| 4a. | When the licence card was issued (not when you first got your driver licence). |
| 4b. | When the licence expires (you must renew it before this date). |
| 4c. | Whether you're interested in being a donor or not (note that your next of kin will still be asked for their permission - make sure they know your preferences). |
| 5a. | Your driver licence number (it's unique to you). |
| 5b. | The driver licence version number (a fraud deterrent measure). The number changes when you're issued with a replacement or renewed licence. Only the most recent driver licence is valid. |
| 6. | Your address (you choose if you want your address shown on the card or not). |
| 7. | The types of driving you're licensed for. The classes are 1 (cars and other light vehicles), 2-5 (trucks, different types of heavy vehicles) and 6 (motorcycles). If the class number is immediately followed by the letter L, then you only hold a learner licence for that class of vehicle. If the class number is immediately followed by the letter R, then you only hold a restricted licence for that class of vehicle. Endorsements are for driving special vehicles (eg, an 'F' indicates forklifts, an 'R' shows that you know how to operate a vehicle on rollers) or working in particular industries (eg, driving instructors have 'I' endorsements, those able to carry dangerous goods have a 'D'). |
| 8. | When the licence classes or endorsements you hold were issued and when they expire. |
| Conditions | Any special conditions that apply when you're driving (eg, that you must wear correcting lenses). |
| 9. C/E | The classes/endorsements to which each condition applies. If there are no classes/endorsements listed, then any special conditions recorded on the licence apply to all the classes/endorsements that you hold. |
